Build critical healthcare integrations. Solve problems most teams avoid.

We’re integrating a modern SaaS product into Pharmacy Management Systems across the US, working directly with our client’s engineering teams.

These systems are:

• Poorly documented
• Inconsistent
• Sometimes decades old
• Mission-critical to real businesses and patients

You’ll be operating across team boundaries, not just inside a single codebase.

This is not a role for someone who needs clean specs and predictable systems.

This is for someone who can walk into ambiguity and make it work.

• Designing and building integration architecture from scratch
• Working closely with client-side engineers to align on APIs, constraints, and system behavior
• Building backend services using TypeScript (Node.js ecosystem) and maintaining/extending parts of the system in PHP
• Making unreliable, third-party systems behave predictably
• Solving problems like:
• Data inconsistencies across systems
• Partial failures and retries
• Sync vs async tradeoffs
• Observability in distributed flows
• Owning problems end-to-end — from unclear requirement → deployed solution

You won’t be handed clean tickets.

You’ll be handed problems that require coordination, judgment, and execution.

• You are exceptionally strong in TypeScript (Node.js)
• You’re comfortable working in PHP codebases (you don’t need to love it, but you shouldn’t avoid it)
• You’ve built or maintained complex systems, not just features
• You’ve worked on integrations, APIs, or distributed systems
• You’re comfortable collaborating with external engineering teams

You understand that:

• “Edge cases” are the real system
• Reliability > elegance
• Debugging is a core skill

You:

• Don’t wait for instructions
• Don’t get blocked by ambiguity
• Communicate clearly across teams
• Don’t ship fragile solutions

We’re likely to prioritize you if:

• You’ve worked with messy real-world integrations (fintech, healthcare, logistics, etc.)
• You’ve collaborated directly with client or partner engineering teams
• You’ve debugged systems where you don’t control all components
• You’ve designed systems that recover from failure, not just assume success
• You think in terms of tradeoffs, not just best practices

• TypeScript (Node.js)
• PHP (existing systems / integration points)
• PostgreSQL
• Async processing (queues, workers, retries)
• Cloud infrastructure (AWS/GCP/Azure)
• Frontend is secondary (React/Vue is enough)

Healthcare standards (HL7, FHIR, NCPDP) are a plus — but the ability to learn fast and adapt matters more.
